    born on ______________________ from Bella's Golden Doodles breeder ( Barbara Pate ) for the sum of ______________
    conditions of sale : 
   Breeder is not responsible for accident, injury, sickness or death after puppy has been released.

   Breeder is not responsible for sickness or death resulting from parvo or other virus.

   Puppy has been dewormed at two, three, four, six and eight weeks of age. Puppy has been wormed, given first vaccination by breeder. Puppy is       healthy at time of release to new home or would not be released.

   Buyer will be responsible for all other vaccinations and medical needs of puppy.

   Seventy-two hours from time puppy is delivered to new forever family, buyer should have puppy examined by a veterinarian to assure health of puppy. 

   In the event that the puppy is determined to be unhealthy by this examination, obtain a signed statement stating the problem. The puppy may be returned for refund or replacement.

   Buyer agrees to have puppy spay/neutered by six to eight months of age.
